LifeWise Individual Policy Holders
For our individual members, LifeWise Health Plan of Arizona will cease being your health plan provider on August 31, 2009. For assistance in choosing a new health plan provider, you may call toll free 1-877-539-9041 to speak with an individual health insurance expert to help you.

LifeWise Group Insurance Policy Holders
For LifeWise group plans, we will offer renewal quotes for existing group customers whose policies renew prior to August 31, 2009, as required by law and issue coverage for one year following the renewal.
Existing group customers with renewal dates on or after September 1, 2009 will not be offered a renewal and LifeWise Arizona will cease acting as their health plan at the end of their policy period. Seventy (70) days prior to the end of the contract, we will send a notice to employers.
